Grace After Fire General Assistance Application Instructions
*******Please do not start the General Application (see link at bottom of the page) until you have uploaded all supporting documents. *****
Auto Insurance:
- Any Name Change documentation
- Any Birth Certificates of children living in your home under the age of 18
- Current Billing Statement
Mortgage:
- Any Name Change documentation.
- Any Birth Certificates of children living in your home under the age of 18
- Current Billing Statement or Ledger with present balance from your Mortgage Company.
Rent:
- Any Name Change documentation.
- Any Birth Certificates of children living in your home under the age of 18
- Current Billing Statement or Ledger with present balance from your leasing office or landlord.
- Full Current Lease (eviction notice if applicable), the lease must have both your signature and the
leasing agency’s signature on it.
- Handwritten note stating where to send payment to if application is approved (payment address)
The Written documentation from yourself should state, "If grant is approved, the payment should
be sent to the following address: (Provide Correct Address given to you from organization requesting
payment). Sign and date the document, scan it in as a PDF and upload it into your profile under
additional documentation and select "W9" category.
- W-9 tax form, completed by your leasing office/landlord.
Utilities (Gas, Electricity, Water):
- Any Name Change documentation

- Current Billing Statement (If your utility bill is behind 3 or more months, please provide the last three billing statements
- Any Name Change documentation
